These forms are required. The Transcript Release Request and the Release Request are available as a download from the College Counseling site or the College Counseling Office. The Request for College Visit form may also be downloaded here; a hard copy is available at the front office.
Transcript Release Request - Transcript requests require that the student complete the Transcript Release Request Form and submit the form directly to the school registrar.
Mid-year reports are sent automatically from the Registrar's office in late January to every school that previously received an official transcript from the school. After graduation, in June, a final transcript is sent to the student's school of choice. Students must notify the College Counseling Office and the Office of the Registrar of their final decision.
Release Request - College Counseling Evaluation Materials - Upon request the College Counseling Office will prepare and mail all required supporting documents for an application. Students must submit a complete Release Request for College Counseling Evaluation Materials Form to college counseling at least three weeks prior to any deadline. A separate form is necessary for each application.
College Visit Request form - This form is available in the front office and is necessary for an excused absence. Please fill out this and return to Mr. LaHaie at least 5 school days before the requested college visit.
Teacher Recommendations
Arrangements for recommendations are between the student and the teacher. Students should request recommendations at least three weeks in advance of the deadline, and provide the required materials, a brief resume, and an addressed, stamped mailing envelop. Recommendations should be mailed by the teacher.
